REINTEGRATION SERVICES
Services Provided to Delinquent Youth Committed to Saint Gabriel's Hall and their Families, Focused on the Development of Community-Based Resources, to Enhance the Treatment Process and Aid in the Transition of Youth from Residential Placement to the Community.

Program Objective:
To prevent further involvement of youth in the juvenile justice system. To help youth be accountable for their actions, develop victim empathy and take greater responsibility for the quality of life in their community. To develop measurable competencies that help youth to become more responsible and productive members of their communities. To assist youth and their families with the re-entry process from Saint Gabriel's Hall to the home and community through the development of community-based resources.

Program Highlights:
- Located within the youth's community in various sections of Philadelphia
- Planning and reintegration services start as the youth is being discharged from Saint Gabriel's Hall and continues up to 6 months following discharge from residential placement
- Monthly visits by a Reintegration Worker with youth and family while the youth is in residential placement
- Participation by the youth's assigned Reintegration Worker in Individual Service Plan meetings and coordination with Saint Gabriel's Hall staff
- Visits by the youth's assigned Reintegration Worker with the youth and families at least once per week once the youth is in the community
- Close collaboration by the youth's assigned Reintegration Worker with Probation in planning and implementation of services provided to youth
- Participation , in the nature of monitoring youth attendance in Reti-Wrap and participation by the youth's Reintegration Worker in discharge meetings, with the youth while in Reti-Wrap, followed by monitoring of school performance and communication with he youth's school by the Reintegration Worker once the youth is enrolled in school
- Referrals provided for youth and family to community resources and supports such as schools and recreation opportunities

- Case management provided to youth and their families to develop resources and supports in their own communities
- Job readiness instruction in life skills, resume writing, job search skills, resources for job training and vocational programs

Referral Information:
Philadelphia youth committed to Saint Gabriel's Hall are automatically referred to Saint Gabriel's System Reintegration Services through the Family Court of Philadelphia at the time of their discharge from the residential program.
